Inquiry forms collect your name, contact details, professional credential, state, experience, interests, and optional message. Model inquiries also collect an adult-age acknowledgment. Please do not submit patient information, medical records, photos, or sensitive health details.
Account and learning information
The site uses WordPress accounts supplied by The Injector League. Account details include your name, email address and a securely hashed password. Sign-in uses an essential session cookie. Your saved inquiries, passed lesson checks and online assessment completion are associated with your account. The assessment is graded on the server; the site saves completion markers rather than a full exam transcript.
How information is used
Information is stored to support your inquiries, account, and learning progress. Inquiry details are also emailed to our designated team inbox so we can respond. This website does not send automated marketing emails, process payments, or transmit your inquiry to a physician or product supplier. Any later external sharing needed to arrange services should be explained before it occurs.
Browser storage
The product inquiry basket and guest learning progress are saved in local storage on your browser. Guest learning records are not automatically transferred to an account after sign-in. Form drafts are saved in session storage in the current tab to support sign-in and accidental navigation.
Access and requests
Your account shows your own inquiry references and learning status. Use the contact form for a request concerning access, correction, or deletion of your information. A public operational contact and retention schedule must be established before public enrollment opens.
Service providers and security
Rocket.net hosts this WordPress website and its account and inquiry database. Our authorized administrators can review inquiries and learning progress. Microsoft Outlook processes inquiry notification emails sent to our designated team inbox. Fonts and brand assets are hosted with this website. No website can promise absolute security; do not use these forms for urgent care or sensitive clinical communication.
